The Importance of Genomic Context in Interpreting Fosfomycin Resistance in Klebsiella pneumoniae

In recent years, we have witnessed advancements in sequencing technologies that allow rapid sequencing of numerous bacterial samples, creating an extensive database of genomes. This information is used nowadays to understand the epidemiological status and trends of resistance, contributing to the change of therapeutic guidelines in the use of antimicrobial agents. For example, the emergence of fosfomycin resistance is of major clinical interest, as this antibiotic is increasingly used, particularly in the treatment of urinary tract infections and sepsis caused by multidrug-resistant bacteria. Recently, new fosfomycin resistance genes have been identified, including novel Glutathione-S-transferases. Since their identification, the origin of most of these genes has been clearly determined. According to Ito et al., the fosA1/2 genes originate from Enterobacter spp., fosA3/4 genes originate from Kluyvera georgiana, and finally Klebsiella pneumoniae is the origin of numerous variants called fosKP that include the fosA5, fosA6 and fosA10 variants.
